Continuing care services, including long-term care facilities, are provided by the Department of Health and Wellness' Single Entry Access (SEA) system. Call the 1-800 number below to speak with a care coordinator who will help you identify your needs (or your family member's needs) for care and the services required, including placement in a long-term care facility. Complete financial and medical information will be collected and you will be placed on a waiting list for the facilities of your choice.
